Referrals:
If a referral is required by your insurance carrier, it is your responsibility to obtain this from your primary care provider. We will be unable to see you without it.
Payment:
Payment is due at time of service including deductibles, co-payments or any balance due. We accept Cash, Checks or Credit Cards for your convenience.
